      Present Illness: Was perfectly well until Christmas 1916 then complained of shortness of breath. Slight cough with it. Food regurgitation. Saur. Not constipated, Continued on light duty at headquarters. In Feb 1917 vomiting continued. Sometimes no vomiting 3 days. Same Pain Rt Side. Noticed distention abdomen all this time was in France then about May 27. Splitting headache. Legs became useless. Warred fall. Had Gonorrhoea 20 ys ago Did not have venereal sore. May 28 Great distension navel protruding. No vomiting for two weeks. Sent here for X Ray. Patient well nourished, ___ natural, Heart natural
